In an effort to enhance medical expertise and alleviate pressure on tertiary hospitals, Xuan Hoa Ward Medical Center in Ho Chi Minh City has launched a strategic training partnership with six major healthcare institutions: Gia Dinh Hospital, the Pasteur Institute, and the Hospitals of Dermatology, Eye, Ear-Nose-Throat, and Traditional Medicine. This collaboration aims to formalize the “Family Medicine” and “Community-based Continuous Care” models.
Rather than waiting for patients to arrive, the medical center is shifting its focus toward proactive and comprehensive health management at the household level. The two-year development roadmap includes:

First 6 months: Standardizing examination procedures for the elderly and completing key professional certifications.

After 12 months: Stabilizing the team-based health management system and strengthening connections with upper-level hospitals.

After 24 months: Establishing a modern primary healthcare model capable of treating complex conditions locally.
Currently, local authorities have dispatched task forces to 24 neighborhoods to update resident health records, conduct chronic disease screenings for the elderly, and provide epidemic prevention training in schools. The appointment of highly qualified personnel, combined with support from leading specialized hospitals, is expected to bridge existing gaps in clinical techniques and medical equipment.
